Ensure
that your Team Application and Individual
Team Member’s Applications were faxed
or postmarked on or before August 6, 2003?

Review the timeline provided (See
Application Guidelines) and tentatively
mark your calendar for October 24-28,
2003?
Attendees must arrive on Friday, October 24 and
depart on Tuesday, October 28. 

Ensure that your team is comprised
of no less than 3, and no more than
5 individuals?

Demonstrate why your collaboration
will be successful in improving the
IAQ in your school and/or school district(s)?

Keep occupational diversity in mind
when forming your team?  Did you
explain the make-up of your team on the
Team Application?

Demonstrate your school or school
district’s need for IAQ assistance
and note any unique school/school district
characteristics on the Team Application?

List any committees that you belong
to or any leadership positions that
you hold?

Include your team’s name and team
leader’s name on the Individual Team
Member Application?

Include your team’s name, team leader’s
name, and each individual team member’s
name on the Team Application?

Review and sign the declaration commitment
agreement on the Individual Team Member
Application?

Mark your top 4 choices (with 1 being
your top choice) of breakout session
topics on the Individual Team Member
Application?
   
  *The
IAQ TfS Symposium will be held October
26-28th. NEA HIN is conducting a pre-Symposium
session on October 25th to address IAQ issues
from the union perspective. This session
is mandatory for NEA members and UniServ
staff attending the Symposium
